For your first visit

What to bring to your first visit

When you are referred to our clinic, the only records we have are what your referring doctor has provided to us.  We do not review your notes before your appointment is made.  Therefore it is in your best interest to make sure you have copies of any pertinent information you want us to review with you. 

  1. Your completed new patient forms – download below
  2. Medication history
  3. Radiology reports pertaining to the area we are to assess.  Do not include CDs of radiology reports as we no longer use computers with CD players.

Please note: It is not our responsibility to obtain your previous medical records.  It is your responsibility to bring whatever is pertinent to your care.  Not having them may delay your care as we cannot assess and treat issues if we have no records or radiology reports to refer to.

 

To make sure your visit goes as smoothy and efficiently as possible, please make sure to fill out your new patient forms BEFORE coming to the office.  Click on the link to the right to download the welcome packet.

For new patients, your appointment time is when you will be seen.  YOU MUST HAVE YOUR FORMS COMPLETED PRIOR TO YOUR APPOINTMENT TIME. 

So if you plan to fill out the forms at the office, arrive 30 minutes prior to appointment time. 

If your forms are not ready by your appointment time, you will most likely be rescheduled.
